How to Get Started
Follow these three simple steps to reserve your spot at Midtown Community Birth Center
Step 1 : Submit Your Application
Fill out the form below to tell us a bit about yourself, your pregnancy, and your birth preferences.
This helps us ensure that the birth center is the right fit for your needs and due month availability.
Step 2: Review + Midwife Selection
Once your application is received, we’ll review it and contact you within a few days.
If you haven’t already chosen an affiliated midwife, we’ll help you connect with one who aligns with your preferences and care style.
Step 3: Secure Your Spot
After your application is approved, a deposit is required to reserve your place on our calendar for your estimated due month (typically 37–42 weeks). Payment in full is due by 36 weeks.
Birth Center Application
Because we keep our client load intentionally small to provide individualized, peaceful care, spaces are limited each month.
